The Health Center achieved many of our goals in 2019 to support our mission.

• We implemented the population health model throughout the entire organization, which employs a patient care team that manages the preventive and chronic health conditions of our patients.

• We created care managers and patient navigators to support our population health initiative.

• We continue to expand our data capabilities to address the link between our patient’s physical well-being and their mental health.

2019 was full of growth and opportunities for Fairfield Community Health Center as we proudly continued our mission of building a healthier community, one life at a time. With 80% of our patient base on Medicaid, Medicare, or without insurance, we played a major role in giving hope and quality healthcare to thousands of individuals who may have otherwise received no healthcare at all.
